Updating Personal Information

To update your personal information on file, we MAY need to have you complete and sign a paper form or you MAY be able to update information through the Employee Portal.

Personal information that can be updated in the Employee Portal:

  • Mailing Address

Personal information that currently requires a paper form with an employee signature:

  • Social Security Number
  • Date of Birth (DOB)
  • Name Change

You can scan, print to file, or even snap a picture of the completed form right from the upload page.
